Non-Emergency Transport

With advance notice, we may be able to schedule a non-emergency transport by ambulance. This generally requires a minimum of 48 hours notice so that we can appropriately schedule a crew to transfer you from one location to the next.

In order for CHS Healthcare to transport a patient “non-emergency”, the following conditions must be met:

  • The patient being transported must be a full-time resident of Henrietta, Chili, Wheatland, Scottsville or Caledonia.
  • The start point or end point must be within one of these towns. For example, if you are being discharged from the hospital and require a transfer home by ambulance.
  • A medical provider (ie. your physician) must provide written authorization for the transfer, stating that the need for a transfer by ambulance is medically necessary. This is done via a “Physician Certification Statement for Non-Emergency Ambulance Services” form (PDF) which outlines the medical condition(s) that require an ambulance to provide transport.

Non-emergency transport by ambulance is only for patients who require one-on-one medical monitoring during the transport process.

CHS Healthcare does not currently provide “stretcher car” or wheelchair transport services. For such needs, we recommend contacting Rural/Metro (585-546-2525), Monroe (585-232-9000), MediCab (585-342-7150), or Rochester Medical Transportation (585-288-3444).

Medicare and most insurance providers cover non-emergency ambulance transport when the service meets “medical necessity” criteria and is certified by the patient’s physician.

To schedule a non-emergency transport by CHS Healthcare, please call us at (585) 334-4190 at least 48 hours in advance and ask to speak with the on-duty supervisor. This service is subject to the availability of personnel and therefore cannot be guaranteed unless scheduled with us.
